Assalamualaikum, This form is to register children for Sunday School program at Islamic Foundation of Toronto.
Timings:

Class timings are 9:50 AM to 2:00 PM every Sunday from September to June.
Registration Fees:

The registration fee is a one-time non-refundable payment payable at the time of registration of $55.
Registration Fees:

1 Child: $55/month

2 Children: $110/month

3+ Children: $150/month

The financial institution identified above is hereby authorized to debit my account for all payments payable to the Islamic Foundation of Toronto in accordance with the tuition fee schedule for the current academic year. I acknowledge and agree that each debit processed under this authorization shall have the same force and effect as if I had personally signed and authorized each individual payment. I authorize the Islamic Foundation of Toronto to collect the applicable fees from my account on the scheduled payment dates for the current academic year. I further agree that the financial institution shall not be liable for any loss, forfeiture, or damage resulting from the non-payment or dishonour of any authorized debit. A copy of this authorization shall be considered as valid as the original, and delivery of this authorization shall constitute delivery by the undersigned.
